At the main menu on controller 2, press A, C, Down, Right, Up, B. If done correctly, a level select menu appears. Select a level with A, B, C, or Start. You return to the main menu. Then when you select 1 PLAYER or 2 PLAYER, the game starts at that level.

Verified using ROM with CRC32 of 2e6eec7e with US console region.
